A new form for participation in Georgia high school athletics has been adopted by the Georgia High School Association, for immediate and mandatory use. It was adopted and put into use on March 17, 2005. A second modification was made, effective 7/1/05. The form is available on our website. If you are scheduled for a sports physical, please complete the history section prior to your visit. Note that the parent must complete the history section, not the student.
